Benefits - Kaiser Permanente 10+ Health Insurance Plan

Temporary contract employees and their families who reside in California can enroll in our special reduced group rate with the Kaiser Permanente 10+ Health Program. You must live within 30 miles of a Kaiser facility. Chancellor & Chancellor will pay 50% of the monthly premium of 180.58 for the primary enrollee; spouses and children may also enroll under the low group rate. Furthermore, the premiums an employee pays for self and family will be deducted pre-tax from their paycheck as part of the Section 125 Medical Cafeteria Plan.

The 2002 rates are as follows:

Temporary contract employees become eligible to enroll after 60 days of employment with Chancellor & Chancellor. To provide ease of coverage for employees, Kaiser Permanente automatically offers roll-over enrollment in their individual coverage program in the event of a change in employers.
You have 30 days from the date of eligibility to decide whether or not to enroll.
Once enrolled, coverage begins as of the date of eligibility.

In the Hospital
All Physician and surgeon services No charge
Room and board No charge
Other necessary services & supplies No charge
Same Day Outpatient care No charge
In the Doctor's Office
Office visits include routine physical exams, hearing and eye exams ( no age or visit limits) $10 per visit
Alcohol and drug dependency care $10 per visit
Emergency room visits $35 per visit
Physical therapy visits (short-term) $10 per visit
Routine well-baby visits (Age 0-2) No Charge
Allergy tests and injection visits 10
Specialty care visits $5 per visit
Laboratory and X-ray
In or out of the hospital No charge
Maternity Care
scheduled prenatal office visits No charge
Hospital services No charge
Prescription Drugs
Up to a 34-day supply No charge
Mental Health Care
Up to 20 visits per calendar year $20
Group therapy $10

Eligible employees and their eligible dependents, who did not enroll when initially eligible, may only enroll during the Open Enrollment Period. The Annual Open Enrollment Period is the month of December and coverage will be effective January 1st.

The Kaiser 10+ Health Plan is one of the benefits offered under Chancellor & Chancellor's Section 125 Plan.
To enroll, simply fill out the Section 125 Enrollment Form

Upon receipt of this enrollment form, Chancellor will send you a Kaiser packet and enrollment card.

You can obtain information on the Kaiser facility nearest you, and about Kaiser in general by viewing their website.

At some point in the future, when your contract comes to an end, you will have certain options
pertaining to your Kaiser coverage.
