Kaiser Permanente 10+ Health Insurance Plan
- End of Contract Options


Now that your contract through Chancellor has ended, Chancellor has to notify Kaiser that you are no longer in the group plan. You have the following options pertaining to your Kaiser coverage:

(1) You may switch to an individual plan. Kaiser will automatically offer you an individual plan called "Conversion".

a) You have a few different individual plan options. You can pay a monthly premium of $130.00 and pay $15 per doctor visit, $5 per x-ray & lab service fee. You can pay a monthly premium of $146.00 per month and pay $5 per doctor visit, $3 per x-ray & lab service fee. Both of these plans have a $7 prescription fee. There is a $1,000 prescription cap. Both of these options do not require a medical review.

b) Once on the "Conversion Plan" you may also apply for the personal advantage plan. This option requires a medical review and your rates are then based upon your age, overall health and locality. An example: a medically reviewed and then approved woman age 30 - 39, living in San Francisco County, pays $5.00 co-payment at $104.00 a month, or $15.00 co-payment and $78.00 a month.


(2) You may end your membership with Kaiser and switch to your new employer's plan.

(3) You may sign up for Kaiser's COBRA plan.


a) This is a provision in the plan that allows for continuation of group coverage when loss of group eligibility occurs due to a "qualifying event" such as end of contract.

b) You may remain on the Cobra plan for up to 18 months at which time you may switch over to any of the individual plan options described in (1) above. However, if Chancellor decides to terminate the group plan with Kaiser, you will not have the option of switching over to an individual plan when your Cobra coverage ends.

In order to make any of the above changes to your Kaiser coverage, you will need to go through Chancellor.
Please fill out the following Request for Continuation Form and fax it us so that we can then send it to Kaiser to make the change effective.
